Treatment will only be conducted if Sara Carr Soft Tissue Therapist deems the treatment to be safe for the client to receive.
In some circumstances, GP or Consultant approval/permission may be requested before treatments can be provided.
A booking is confirmed once Sara Carr Soft Tissue Therapist and the client have both agreed a date and time for the appointment.
Payment is required in advance of the appointment, via online bank transfer, details will be provided with a confirmation email of the appointment.
I value your time, and in the unlikely event that I need to cancel your appointment I will endeavour to give you as much notice as possible.
I ask that should you need to cancel an appointment, that you give me 24 hours notice so that I can offer up your appointment to another client.
Please note that appointments cancelled with less than 24 hours notice will be charged in full.

If a client is more than 15 minutes late for an appointment, Sara Carr Soft Tissue Therapist has the right to cancel the appointment. No additional time will be given if a client arrives late for an appointment. The appointment will still finish at the scheduled time and the full fee will still be payable.
Please do not come to your appointment if you are ill. I appreciate 24 hours notice, so that your appointment can be offered to another client. This is for your benefit, and mine and I am always happy to rebook in the future when you are better!
People of all ages can benefit from Soft Tissue Therapy. Under 18's must be accompanied by an adult for all appointments.
Sara Carr Soft Tissue Therapist has Professional and Public Indemnity Insurance for all treatments offered with Balens Insurance.
Sara Carr Soft Tissue Therapist reserves the right to refuse to treat anybody showing signs of aggressive or inappropriate behaviour during an appointment. In such circumstances you will be asked to leave the premises, and full charge will still apply.
Consultation forms and treatment notes will be kept securely on file in line with GDPR regulations.

I understand that the Soft Tissue Therapist is providing treatment within their scope of practice, as defined by the ISRM (Institute of Sports and Remedial Massage). I hereby consent for the therapist to treat me for the purposes disclosed, including such assessments, examinations and techniques which may be recommend by my therapist. I understand that the therapist is not a Doctor, and cannot diagnose, and the the following information is required by the therapist for my safety. The following information will be treated in the strictest of confidence. It may, however, be necessary for you to consult your GP before any treatments can be given.

Client details will not be discussed with a third party without prior consent of the client.
By signing the consultation form, the client is giving Sara Carr Soft Tissue Therapist permission to hold records and data about them.
In order to comply with the General Data Protection Regulation (GDPR), I need to inform my clients how I handle their data.
The personal data held is information they have provided to me on my Client Consultation Form: name, address, date of birth, email address, GP details, basic medical history. This information is not shared with anyone else, and is stored in a file in a locked cupboard in my home.
The medical data I hold is only used for the purpose for which it was collected, which enables me to form an informed opinion on my clients' medical background. An email address is held on mailchimp, which is a secure site which follows GDPR rules. My clients will only ever receive an email from me that is totally relevant to their interests and needs. Clients are able to opt in to receiving specific emails from me, such as appointment reminders and newsletters.
By law I am required to store client data for 7 years from their last appointment, or date the notes were taken. For clients under 18 years of age, I am required to store their data until they are 25 years of age.
Clients have the right under the Data Protection Act 2018:
A client may exercise their rights at any time without charge.
